This past year has been a busy time for OnTrack. Like many behavioral health providers across Oregon, we’ve worked diligently to adapt to changes in Measure 110, the workforce shortage, the end of pandemic relief funding, and legislative efforts addressing the state’s behavioral health and housing crisis. Despite these challenges, we’ve made significant
progress.

This past year we hired more staff, expanded services, and increased our capacity to serve the Latino community. Our emergency lodging programs had a profound impact, providing safe temporary housing for people to those living on the streets and awaiting residential treatment. In September 2023, we partnered with Oasis to open six emergency lodging beds, followed by a 10-bed complex in February 2024. Since then, we’ve helped 139 individuals transition from homelessness to treatment.

In 2025, we look forward to completing construction on our 12-bed emergency

lodging facility and outpatient treatment program in Grants Pass, opening this spring. Additionally, renovations for the Sis-Q Motel in Medford will add another 12 emergency lodging beds.

We are also beginning renovations on Cobblestone Village to establish a Family Treatment Campus. This project will provide residential treatment for parents and young children, on-site licensed childcare, and physical healthcare services through our partners at Oasis. The campus will offer family-centered treatment to break the intergenerational cycle of trauma, poverty, and substance use.

Our staff’s dedication inspires me daily as they help individuals rebuild their lives and find  stability. Collaboration with our community, county, CCOs, and law enforcement makes this work possible.

Partner Recognition

1. Family Treatment Campus – Expanding Family-Centered Care

Our vision of creating a place where families can heal and grow together is becoming a reality. Through the collaborative efforts of ODHS, OHA, and JCC, we are transforming a 2.7-acre campus into a whole-family treatment center that provides critical support and interrupts the intergenerational cycle for families navigating recovery.

Beyond securing essential funding, this project is shaped by the expertise and dedication of our partners. Early collaboration with MASS Design Group and their referral to the amazing team at Mahlum Architects has ensured a thoughtful, trauma-informed design, while Adroit Construction’s significant in-kind contributions have been instrumental in bringing this vision to life. Their expertise, combined with the unwavering commitment of our funding partners, is making it possible to create a facility that includes licensed childcare, outpatient and residential treatment, family reunification support, and integrated medical and mental health care—all within a welcoming environment.

The impact of this project extends beyond the campus itself. By relocating existing programs to this new site, we are creating opportunities to repurpose current facilities for new initiatives, such as a culturally specific residential treatment program for Latino and a co-occurring treatment facility for individuals facing both substance use and mental health challenges.

We are deeply grateful for the collaborative partnerships that are making this vision a reality. Together, we are building a space where families can rebuild, reconnect, and thrive on their journey to recovery.

2. Sis-Q Emergency Lodging – A Bridge to Stability

Recovery begins with a safe place to rest, and thanks to JCC’s support, that vision is becoming a reality at Sis-Q. In 2024, their $680,000 contribution allowed us to begin transforming a 12-unit former motel into an emergency lodging for individuals who are unhoused and awaiting detox or residential treatment.
Renovations will include ADA-compliant rooms, enhanced security, and comfortable, dignified living spaces. Once complete, the Sis-Q site will provide immediate support for 250–300 individuals annually, connecting them with services and support while they wait for treatment beds. We extend our heartfelt thanks to JCC for believing in the power of stability as a foundation for recovery.

3. Emergency Lodging – Grants Pass: Pathway to Recovery

A safe, stable place to stay can be the turning point between crisis and recovery. Thanks to the collective generosity of M110/OHA, the City of Grants Pass ($890,000), AllCare ($215,000), and the Four Way Foundation ($8,000), we are establishing an emergency lodging facility that will bridge the gap between temporary housing and long-term treatment. This facility will provide immediate lodging and essential resources, giving individuals in need the opportunity to rebuild their lives.
We are deeply grateful to our supporters for bringing this vision to life. Their generosity is creating hope, stability, and a future filled with possibilities for those in need.
