Outpatient Substance Use, Mental Health, and Co-Occurring Treatment
Programs and services provided in Jackson and Josephine Counties
OnTrack Rogue Valley Outpatient Treatment provides substance use, mental health, and integrated co-occurring treatment for clients assessed at the following American Society of Addiction Medicine (ASAM) levels of care (LOC):
- 0.5 - Early Intervention (DUII and MIP)
- 1.0 - Outpatient (Substance use, mental health, and co-occurring conditions)
- 2.1 - Intensive Outpatient (Substance Use)
- 2.5 - Partial Hospitalization/Day Treatment (SPMI & SUD)
Our outpatient treatment services are individualized and based on the unique goals, strengths and needs of each person.
Our service locations are accessible, trauma informed, and respectful. We are open evenings and Saturdays to meet the needs of working families. Our group and individual counseling services utilize evidence-based practices designed to teach skills and support individuals on their path to wellness and recovery.
We accept all major insurance and have Oregon Health Plan (OHP) assistors available to help.
Recovery 360
New Program Launched Summer 2023
Medford, OR
Recovery 360 is a day treatment/partial hospitalization program (2.5 ASAM LOC) for individuals with severe, persistent mental illness (SPMI), substance use disorders (SUD), and forensic involvement. Participants will be on-site at OnTrack for 35 hours a week of structured whole-person activities. The structure includes physical activities, mindfulness, art/recreation, treatment groups, contingency management based on UA’s and a separate token economy based on engagement and participation.
The day treatment program is co-facilitated by Jackson County Mental Health (JCMH) and OnTrack to leverage the expertise of both agencies. Clients will be engaged with and get lunch at Compass House. As clients build connections with Compass House, the day treatment structure will include options to participate in recreational and skills training activities at Compass House or to remain on-site and engage with OnTrack and JCMH staff. Paradigm Mental Health and Wellness will be on-site providing medication management and medical support.
Recovery 360 is a collaborative partnership among OnTrack Rogue Valley, Jackson County Mental Health, Jackson County Community Justice, Compass House, and Paradigm Mental Health and Wellness.
Recovery 360 referrals are made through Jackson County Mental Health.
